One such challenge is iron deficiency, a prevalent issue in Bangladesh and a leading cause of anemia. But here's where it gets controversial: the impact of iron deficiency extends beyond health, affecting education and quality of life, especially for adolescent girls.
The Iron Deficiency Puzzle
Dr. Hasan and his team have been conducting iron intervention studies for several years. Now, they're shifting their focus to heavy menstrual bleeding, another major contributor to anemia in women of reproductive age. This issue is more widespread than one might think, with a recent report indicating that a staggering 41% of Bangladeshi adolescent girls miss school due to heavy menstrual bleeding. It's a problem that significantly impacts their daily lives, yet there is currently no standard treatment protocol.

The Partnership's Roots
The collaboration between icddr,b and WEHI began with the BRISC (Benefits and Risks of Iron Interventions in Children in Rural Bangladesh) clinical trial. This trial assessed the impact of iron syrup and micronutrient supplements on child development. It was followed by EDIVA (Efficacy and Demonstration of IntraVenous Iron for Anaemia in Pregnancy), which compared intravenous and oral iron treatments for anemia in pregnant women.
A New Chapter: The TXA Clinical Trial
Most recently, the partnership received a significant boost with a $4 million grant from the Medical Research Council in the UK. This funding will support a clinical trial testing oral tranexamic acid (TXA), a safe and low-cost medicine that reduces bleeding and improves hemoglobin levels. The trial aims to demonstrate that oral TXA is more effective than a placebo in improving hemoglobin concentration and menstrual-related quality of life.
Dr. Hasan's role is pivotal as he leads this trial in Bangladesh, overseeing ethics approvals, staff recruitment, training, laboratory setup, and community engagement. It's a complex task, but one that the team is well-prepared for, having successfully navigated similar challenges in the past.

Heavy menstrual bleeding, a topic often overlooked in global health research, is a key focus of this collaboration. It's a problem that affects up to half of all women in low- and middle-income countries, yet it receives little attention. This issue drives anemia and disrupts education, particularly for young women. TXA offers a culturally sensitive solution, as it directly reduces bleeding without hormonal effects, unlike contraceptives.
The TXA Clinical Trial: A Glimpse into the Future
The clinical trial is set to begin in early 2026. Girls aged 15 to 17 with heavy menstrual bleeding will be randomly assigned to either the TXA or placebo group and followed for 12 months. The study will measure hemoglobin levels and quality of life to determine if TXA makes a significant difference.
As a senior research officer, Dr. Davidson's role is crucial. She coordinates between the field team in Bangladesh and the researchers in Melbourne, monitoring data and ensuring the smooth operation of the trial. She works closely with Dr. Hasan and the statisticians, managing all the necessary approvals in Australia.
